Our mission is to provide family-centered, community-based, culturally competent, coordinated care for children with special health care needs in managed health plans through consumer/provider partnerships. Our objectives are as follows...
  • To establish a new position in hospitals called Parent Liaison and to create Parent-Directed Family Resource Centers at the health care sites which will provide PHP's services to families and professionals.
  • To provide information and training for doctors, nurses, parents and professionals including the elements, principles and indicators of family-centered, culturally competent, collaborative coordinated care.
  • To provide emotional support to parents and professionals via parent-to-parent and professional-to professional mentors.
  • To provide outreach to typically underserved communities.
  • To provide a computerized directory of health care resources (PHP Online.)
  • To evaluate the impact of the Parent Liaison on family-centered, culturally-competent care.
